The purpose of this reflection is to look at the impact of assessment on care planning and the subsequent healthcare delivery based on its outcomes. The reflective model I am going to apply to this reflection is Gibbs model (Gibbs 1988). The author of this assignment is currently a second year Trainee assistant practitioner (TAP) working in an Inpatient Assessment and Treatment (AT) unit for adults with a learning disability and/or co-morbidity. In accordance with the skills for Health (2013) Code of professional conduct, confidentiality shall be maintained and the names and places will remain confidential and pseudonyms (*) will be used, for the purpose of this reflection the patient will be referred to simply as†Lizzie†. Description My current place of work is an inpatient Assessment and Treatment (AT) unit for adults with a learning disability and/or co-morbidity. I work within a large inter-professional team who all provide input into the patients care and treatment, the unit provides care for both male and female patients with a variety of complex and specialist needs, most of the patients being detained under the Mental Health Act (1983).
